A reminder an online notification has been created for RMOs / SMOs to notify GPs their patient has died in the ward or the Emergency Department.
This has been requested to enable early notification of a patient’s death when the discharge summary may take a while to complete by the parent team.
Early notification to the GP enables primary care to provide early support to distressed whānau.
Using this notification does not replace the need for a formal discharge summary.
If there are cases where this hasn’t worked, please contact Dr Mark Taylor, Te Whatu Ora Clinical Lead GP Liaison Team, so feedback can be given to the relevant team.
