This programme is designed to support proactive management for people with chronic respiratory conditions to optimise patients’ chances of staying well during winter months. Funding is available for comprehensive reviews and care planning for patients who are at high risk of respiratory exacerbation during the winter months. This programme is designed as planned care, not for acute presentations.
Please note this funding is only available for care provided between 1 April and 30 June 2026.
